Make an appointment

To obtain a medical dermatology appointment with one of our dermatologists, you must have obtained a medical referral from another physician. Your physician can submit the referral to the CRDS (Service Request Distribution Center) or send it to us by fax or email. You can also send us the referral yourself by fax, email, or in person.

 

Once the referral is received, it will be assessed based on the urgency of the reason for consultation, and our team will contact you to schedule an appointment.

Please note that no matter which method you use to send us your referral, none will get you an appointment faster than any other.
